Greetings!

The registration has been categorized as:
1) INDIVIDUAL REGISTRATION (ONLY FOR QUALIFIED PHYSIOTHERAPISTS)
2) GROUP REGISTRATION (ONLY FOR STUDENTS). Students shall register through their respective colleges and details shall be sent from college Principal.

Google Form is having three sessions: REGISTRATION LINK

SECTION: 1-  TO FILL THE REGISTRATION TYPE AND PAYMENT DETAILS FOR ALL.

SECTION: 2- This section of the form is only for Individual registration for Qualified Physiotherapist. OR for in-charge staff in case of group registration  for students.

SECTION: 3- This Section is only for Group/Student registration.  

-->INSTRUCTIONS FOR INDIVIDUAL REGISTRATION (ONLY FOR PHYSIOTHERAPISTS):
1. Complete the payment of registration fee via-UPI/NEFT/IMPS to below mentioned conference account. The payment made towards conference is non-refundable.
2. Complete this online registration google form REGISTRATION LINK section 1 and 2 only, do not fill the  section 3. Kindly check all the details before submission. You cannot make changes once the form is submitted.
3. Once details are verified by the registration committee, the committee will sent the conference registration number by an email.

4. To attend the conference you shall bring any Government issued original photo id proof, preferred is Aadhara. 

-->INSTRUCTIONS FOR GROUP REGISTRATION (ONLY FOR STUDENTS):

1. The registered student details for group registration shall be entered in excel format DOWNLOAD THE GROUP REGISTRATION FORM. Please rename the file with your college name.
2. Before filling this form, complete the payment of registration fee via-UPI/NEFT/IMPS to below mentioned conference account. The payment made is non-refundable.
3. Fill the payment details in section 1 of this form REGISTRATION LINK.
4. Fill the details of in-charge staff assigned by the College Principal in the section 2 of this form.
5. Shall submit the registered student details for group registration in excel format DOWNLOAD THE GROUP REGISTRATION FORM and submit the details in the section 3 of this form.
6. Once details are verified by the registration committee, you will receive the conference registration number by an email.
7. To attend the conference students shall bring college issued student identity card.  Attendance to conference for two days is mandatory to receive the certificate.

1. Any payment made towards conference is non-refundable.
2. Registration stands invalid if the form is filled with incorrect information and if no payment made towards registration.
3. Registration includes: Entry fee for two days, Food and refreshments for two days, Conference kit.
4. It is mandatory to wear conference ID card or conference wrist tag whichever is provided during the two days of conference.
5. Attendance to conference for two days is mandatory to receive the certificate. Conference kit and certificate will not be issued to absentees and conference fee paid by absentees is non-refundable.
